Practice Transformation: Lessons Learned from Multiple Integration Projects

Practices implementing integrated behavioral health and primary care require transformation on multiple levels in order to be successful. Implementation of elements of the patient-centered medical home model is necessary, along with dealing with culture change and culture clashes. The result is improved and more accessible health care for our patients — but the path to this transformation can be challenging. This webinar will present lessons learned from multiple integration projects regarding this transformation process.

This webinar is presented by The Office of Integrated Healthcare Research and Policy in the Department of Family Medicine at the University of Colorado School of Medicine.
