How We Meet

Board Committees

Special Interest Groups

Workgroups

Commmunity Conversations

Board Committees

The Board of Directors oversees two committees dedicated to upholding CFHA’s mission: the Research and Evaluation Committee and the Just Medicine Committee.
These groups play a critical role in ensuring our work remains evidence-informed, equity-driven, and aligned with our organizational values.

Special Interest Groups

CFHA’s Special Interest Groups (SIGs) offer members the chance to connect, collaborate, and lead in key areas of integrated care. Each SIG is member-driven and focused on advancing knowledge, practice, and innovation while staying grounded in CFHA’s mission and values.
Current SIGs include:

  • Primary Care Behavioral Health SIG
  • Families and Health SIG
  • Pediatrics SIG
  • Collaborative Care Model SIG

Workgroups

Our workgroups are mission-driven teams dedicated to advancing specific initiatives within integrated care. Each workgroup is formed around a focused goal, with clear endpoints or deliverables expected as the group’s outcome. These groups require committed leads and consistent participation from members over a set period to ensure progress and impact. Unlike our ongoing groups, workgroups are time-limited, disbanding once their objectives are met.

Starting or joining a workgroup is an excellent way to contribute your skills and make a measurable difference in a targeted area, all while collaborating closely with like-minded professionals.

View our workgroups here. If you are interested in starting or joining a workgroup, email us at info@cfha.net.

Commmunity Conversations

Community Conversations offer integrated care colleagues a relaxed space to discuss high-interest topics, share experiences, and ask questions. As an entry-level and informal way to engage, these meetings encourage meaningful exchanges on a wide range of subjects. Not sure about leading the conversation yourself? Our Community Conversation Facilitator, Stacy Agosto, is available to help frame and guide discussions. We also frequently invite “Super Participants” — subject matter experts — to enrich these conversations and make it easy for members to get involved.

Have an idea for a Community Conversation topic? Email Stacy Agosto at stacyagostolcsw@gmail.com to learn more and get a conversation scheduled! You can also check out our calendar for future Community Conversations here or view old recordings here.

Our Structure

Board of Directors

There are up to 15 Board members (either appointed or elected CFHA members), in addition to ex officio members. Each Board member brings unique expertise and perspective to guide CFHA’s strategic direction and uphold its mission.

Meet Our Board
