For the past 20 years, CFHA conference attendees from diverse clinical and professional backgrounds have consistently explored how to achieve the true practice of the biopsychosocial systems model through collaborative care. From integrating behavioral health into primary and specialty care, to enhancing patient and family centered care, and to promoting team functioning, CFHA has led clinical innovations, pioneered cutting edge research, while steering health care policy toward comprehensive care for all people.  

Moving forward with healthcare reform, this conference will continue to showcase ways to help healthcare teams successfully implement best practices, improve patient safety and outcomes, and engage systems to achieve these goals. The 2018 conference will celebrate CFHA’s rich history while exploring, and championing, solutions to current health care challenges.

The conference will include programming in innovative areas of collaborative family healthcare, especially highlighting (though not limited to) the following:   

1) INTERPROFESSIONAL TEAMS: Helping clinicians of different disciplines work together to achieve better healthcare, such as increased medication compliance, better diabetes management, or decreased depressive symptoms following a heart attack. This year’s CFHA conference will provide a systems lens looking closely at team initiatives that demonstrate best practices, exploring how to teach teams, interprofessional education, communication, and evaluating cutting edge team development.

2) ACROSS THE LIFESPAN: Integrated teams can effectively help patients of all ages and their family members who take on significant caregiving responsibilities. This emphasis will increase attendees’ discussions on issues prevalent within systems treating children through older adults -- from ADHD, caregiving of the disabled or elderly to end of life issues.

3) OPIOID CRISIS & SUBSTANCE USE MANAGEMENT: We want to pay special attention to innovations that can help address the Opioid addiction epidemic. Programs that use a systemic or team-based approach are especially encouraged to apply.

To achieve these goals, we will bring together clinician, research, administrative and policy innovators across disciplines to think creatively, exchange ideas, problem solve, and inspire change.

More than 500 attendees are expected to attend the 2018 CFHA Conference in Rochester, New York.

One-Day Registrations (Friday or Saturday)

All fees increase after September 21.

 

 CONTINUING EDUCATION

CFHA will provide CEU's through an outside company, Amedco, and CME through the AAFP. Check back for additional information.

   

FAQ

How do I find out about membership?

Use this link to learn more about membership or to become a member. Membership costs at most $225. It is less expensive to become a member first and then register for the conference.

 

Who has to pay?

Everyone needs to pay to attend the conference. All presenters, co-presenters, panelists, discussion group facilitators and poster presenters are required to register to attend and present at the CFHA Conference. There are no discounts or fee waivers for conference presenters.

 

Do you have scholarships?

A limited number of volunteer positions are available for students. The registration fee is waived for volunteers. Applications are available here. Please note, volunteers must give a $100 deposit that is returned when they complete their volunteer hours.

 

Do you have any registration fee discounts?

The first 50 students, MAs, or CASACs who are CFHA members get a $100 discount. Use the promotion code First50Student on the payment page.
 

PAYMENT 

Payment is required in U.S. dollars by check or major credit card.

If you pay by check, please issue payment to "CFHA Conference" and mail within five (5) business days to the CFHA office. Be sure to include the name of the registrant with your payment.

Institutional purchase orders will also be accepted and PO number must be referenced with registration.

All payments must be received by CFHA no later than October 1, 2018.

CANCELLATION POLICY AND REFUNDS

Substitutions are encouraged in lieu of cancellation. Please contact CFHA’s Conference Manager via email to report substitution.

All cancellations and refund requests must be reported in writing:

Refund requests received on or before Friday, September 28, 2018 will receive full refund less $30 processing fee.

There are no refunds for cancellations after Friday, September 28, or no shows.
