Registration Details

Online Conference registration: June 15 through October 10, 2014

  • Early bird registration discount fees are available through September 15, 2014.
  • Advance registration fees will apply from September 16 through October 10.
  • Online registration will close at 5:00 PM (Mountain) on Friday, October 10th.
  • After October 10, you will be required to register Onsite and may be subject to limited options.
  • Onsite registrations are subject to higher registration fees.
  • Onsite registration begins at 2:00 PM on Thursday, October 16th at the Omni Shoreham Hotel in Washington, DC. 
  • Full payment will be required at the time of Onsite Registration.

What is included in the Conference registration?

Full Conference registration includes admission to the following functions:

  • Plenary sessions on Thursday , Friday and Saturday;
  • All concurrent education sessions on Friday and Saturday;
  • Access to CFHA Lounge with exhibits, posters, etc.;
  • Welcome Reception on Thursday evening;
  • Discount coupon for CFHA Cafe breakfast on Friday and Saturday;
  • Lunch and refreshment breaks on Friday and Saturday.
Please note:
  • Discounted pricing is available for CFHA Professional Members and Student Members. You must log-in to the website before registering online to apply the Member price.
  • Pre-Conference activities on Thursday are NOT included with Conference registration; additional registration and fees will apply.
  • Dinner is NOT provided during the Conference.
  • A spiral-bound Conference Program Workbook will NOT be included for Conference registrants. Materials will be available for free via download from the CFHA website or through a convenient mobile app. You can pre-order a a Conference Program Workbook when you register for the Conference. Quantities are limited; workbooks will be sold for $25 each while supplies last.

 One-Day Registration

One-day registration is available for Friday, October 17 and Saturday, October 18, includes admission to functions on the specified day only:

  • Daily plenary session on Friday or Saturday;
  • Daily concurrent education sessions on Friday or Saturday;
  • Access to CFHA Lounge with exhibits, posters, etc.;
  • Discount coupon for CFHA Cafe breakfast on Friday or Saturday;
  • Lunch and refreshment breaks on Friday or Saturday.

Registration on Thursday, October 16, is required for Pre-Conference activities.

  • Thursday registration includes Welcome Reception and Opening Plenary Session on Thursday evening.
  • Registration for full-day or half-day workshops are available.
  • Pre-Conference workshops are NOT included within the Conference registration fee; additional fees apply based on your workshop option.
  • Lunch is NOT provided for Pre-Conference Workshop attendees.


What are Pre-Conference Workshops? 

The CFHA Conference will include three educational activities on Thursday, October 16, 2014, prior to the start of the CFHA Conference.

Pre-Conference Workshops are in-depth, full or half-day sessions offered as an optional registration with additional fee. Workshops typically focus on specific or unique skills and best practices.

These workshops are NOT included in the Full Conference registration fees; advance registration is required. Full-day and half-day workshops are available; lunch is NOT provided with these workshops.

I'm presenting a session at the CFHA Conference. Do I need to register?

Yes, all CFHA Conference presenters must register to attend the CFHA Conference. No presenter discounts or fee waivers are available.

You can register for the Full Conference or select a One-Day registration option for the day of your presentation.

Please register before Monday, September 15th for early bird discounts.


How do I register for the CFHA Conference?

Online registration opens on June 16, 2014. Click here to register online now.

If you're a CFHA member or have attended a previous CFHA Conference, please sign-in first using your user ID or email address. Your CFHA membership must be current to register at the specially priced Member registration fee.

If you are not a member, click on the appropriate links for Non-Member Conference registration.

If you are registering 5 or more individuals from the same institution at the same time, you may Register as a Group. No discounts will apply for groups, however, you can streamline the data entry process and pay for all registrations in a single transaction.

A registration confirmation will be sent via email prior to the CFHA Conference.


How much are Conference registration fees?

You can register for the Full Conference (Thursday evening through Saturday), or select a one-day option for Friday or Saturday.

*A limited number of Student registrations will be accepted for the 2014 Conference and CFHA Members will have first priority. To qualify as a Student, the individual must be enrolled in a full-time, institution-based university, residency, internship, or fellowship program with a set completion date.

Please note:

NEW EVENT! Monuments by Moonlight Trolley Tour ($50 per person)

You haven’t seen Washington until you’ve seen it at night!Enjoy a night of moonlight merriment aboard Old Town Trolley Tours. A professional tour guide will take you right to some of the city’s most popular monuments and let you see the city in a different light, stopping at Iwo Jima Memorial, FDR Memorial and the Lincoln Memorial. Along the way, you’ll be transported back in time as you ride along the same streets the presidents have traveled, and hear historical tales and anecdotes about the city’s fascinating history. This 3-hour private tour for CFHA Conference participants departs on Friday evening from the Omni Shoreham Hotel at 7PM and returns at 10PM.  Capacity is limited and advance tickets are required.

Tickets can be added when you register for the Conference, or purchased separately through our online store.

How can I get a Conference Program Workbook?

CFHA is going green for the 2014 Conference in Washington, DC. In lieu of publishing a spiral-bound Program Workbook for Conference registrants, Conference materials will be published online at 

The contents of the Program Workbook will be available for FREE as a PDF download through the CFHA website before and after the Conference. A convenient mobile application will also allow you to view details online through your mobile device.

The Program Workbook contains:

  • A detailed agenda of daily Conference sessions;
  • Map of the Conference hotel;
  • Detailed session descriptions with presenters and learning objectives for Plenary Sessions and Concurrent Education Sessions;
  • A list of Poster Presentations;Information about local events and activities;
  • Lined pages for note-taking; and
  • A list of Conference registrants. 

If you still prefer the tangible format, you can PRE-ORDER your copy of the Program Workbook for $25 each until September 26th.

A limited quantity will be available for sale for $25 each while supplies last.

To pre-order your workbook, select "2014 Conference Program Spiral-bound Workbook" under the Optional items section when you register for the Conference.




Full payment is required prior to the Conference. Payment will be accepted by major credit card (preferred) or check payable to "CFHA Conference".

Institutional purchase orders will also be accepted and PO number must be referenced with registration. All payments must be received by CFHA no later than October 1, 2014.


Cancellation and Refund Policy


Substitutions are encouraged in lieu of cancellation. Please contact CFHA's Conference Manager to report a substitution.

All cancellations and refund requests must be reported in writing to:

  • Fax to 303-395-2609
  • Mail to Collaborative Family Healthcare Association, P.O. Box 23980, Rochester, NY 14692-3980.

Refund requests received on or before Friday, September 26, 2014, will receive full refund less $30 processing fee.

There are no refunds for cancellations after Friday, September 26. 


Special Needs

If you require auxiliary aids or special assistance service to attend this activity, please specify your assistance needs on the registration form when you register for this Conference.




Registration Scholarships

CFHA will be offering Student Members and New Professionals the opportunity to register for the 2014 Conference at low or no cost registration fee.

A limited number of scholarships will be awarded to current CFHA Members on a first-come, first-served basis to applicants who meet the required criteria. Please note that scholarship is for registration fees only; it does not include travel or accommodations. 

The scholarship application period for the 2014 Conference has closed.

Scholarship Contributions

Members are encouraged to contribute funds toward registration scholarships for Students to attend the CFHA Conference. A $100 contribution is suggested; any amount is appreciated.

Scholarship contribution can be added to your Conference registration.


Resource Materials

In lieu of printed handouts, CFHA Conference registrants may access resource materials on the CFHA web site.

When you register for the CFHA Conference, you will be required to create a web site log-in. You will need to use this log-in to access Conference resources on the CFHA web site.

Non-member registrants will be given online access to 2014 Conference Resources for a limited period of time. CFHA members may access these resources by retaining membership.
