8:00 AM to 3:00 PM      PCBH Community Forum

This event, co-sponsored by the CFHA and Cornerstone Whole Healthcare Organization (C-WHO), will be the first large professional gathering focused specifically on the PCBH model of integrated care, one of the most widely used models of integration. The goal for the day is to organize, challenge, inform, entertain and energize the PCBH-interested community. It is open to all levels of interest and expertise.

The main focus of the day will be discussions led by national experts, top tier researchers and proven on-the-ground clinicians regarding the keys to better understanding, implementing and disseminating the model. We will also learn about the history of the PCBH model, as well as recent developments pertinent to the model, such as coming changes to APA’s ethical standards and notable new/in-progress PCBH research. A mix of presentations, panel discussions, trainings, and workgroups will be used throughout the day. In short, this event intends to engage participants and leave them with a better understanding of the model, in order to move forward both the field and their own PCBH work.


This event occurs right after the annual CFHA Conference in Denver, CO (Sheraton Downtown Denver), October 17-19, which is a separate registration process.

Opportunity for Training in Research and Evaluation at the Conference

Looking for practical information about demonstrating patient outcomes? Need help deciding how to evaluate your program and its effects? We have just the resource for you! We are pleased to announce that the Research and Evaluation Training Track is back by popular demand for the 2019 CFHA Conference!

This track comprises a series of presentations designed to equip CFHA members with practical knowledge and skills. The track will provide “how to” lessons regarding how to plan and conduct research, quality improvement, and program evaluation in real-world health care settings. This year’s track is bigger and better than ever! Those who attend and sign in for 2 or more sessions in this track will receive a CFHA Research and Evaluation Training Certificate as recognition for their efforts to gain skills in research and evaluation methods.
