CFHA Award Winners 

 

We are honored to recognize the exceptional achievements in patient and family-centered integrated collaborative health care modeled by each of our award winners.  We celebrated each individual and organization at our 2019 conference, Our Generation Integration in Denver, Colorado October 17-19th, 2019.


Natalie Levkovich

 

DON BLOCH AWARD

Natalie Levkovich has been inspired by, and continued the mission, of Don Bloch in the domains of intellectual, behavioral, and relational impact on healthcare. She has been a strident leader in improving health care with her innovation and advocacy, and this has had a predominant impact on the membership Collaborative Family Healthcare Association. Ms. Levkovich serves as the Executive Director of the Health Federation of Philadelphia and has had a role in leadership in this organization since 1984. The Health Federation of Philadelphia serves as a consortium of community health centers in Southeast Pennsylvania, providing care for over 300,000 individuals in high health disparity areas. This organization has been a regional leader in driving health care transformation and improving the quality of life and outcomes of the community.

 

OUTSTANDING CONTRIBUTIONS TO THE PCBH MODEL AWARD 

This award recognizes individuals who have made outstanding contributions to health care through their commitment and use of the Primary Care Behavioral Health (PCBH) model. Community Health of Central Washington team has developed and initiated numerous programs, projects, and ideas to help fortify the primary care services at CHCW. CHCW is where PCBH happens.

RESEARCH AND PROGRAM EVALUATION FELLOWSHIP

This competitive award supports the work of a trainee or early career professional whose research project significantly contributes to the field of integrated care. CFHA is proud to support Julie Gass' project entitled Primary care to order: adapting the Customized Care intervention to improve team communication for patients with multimorbidity to integrated care settings.

What We Do

CFHA is a member-based, 501(c)(3) non-profit organization dedicated to making integrated behavioral and physical health the standard of care nationally. CFHA achieves this by organizing the integrated care community, providing expert technical assistance and producing educational content.
