ELO 3: Collaborative Care: An Advanced Implementation Workshop

Stuck in your implementation efforts for CoCM (Collaborative Care Model)? Wondering how you can take you CoCM work to the next level? Well this session, led by AIMS Center psychiatrist and world-leader in the CoCM world, Dr. Jurgen Unutzer is for you. The Collaborative Care Model will be briefly reviewed before attending to the keys to successful implementation and moving toward your 2.0 version of the model. The discussion will center around the best implementation evidence out there and efforts to move the model beyond depression to the co-management of other prevalent conditions in primary care.

Objectives: At the conclusion of this presentation, participants will be able to…

  • Appreciate and articulate the value of implementing an evidence-based collaborative care program in primary care.

  • Develop a training and implementation support program for unique primary care settings.
