Over the last two decades, studies have shown that integrating behavioral health and primary care services can improve mental health and physical health outcomes, reduce substance use, and decrease inappropriate service utilization and associated medical costs. The passage of the Patient Protection and Affordable Care Act (ACA) and the emergence of more comprehensive approaches to care such as the Patient Centered Medical Home (PCMH) have facilitated the pursuit of the Quadruple Aim of better health, better care, lower per capita costs, and greater provider satisfaction.
The goal of the 2017 Conference is to promote the continued development, implementation, and evaluation of integrated health care while considering its evolution and future. The framework for the conference will be anchored in the 4 Ps: Practice, Programs, Policy, and Partnerships. Attendees will learn about the full continuum of integration through best practice presentations and policy discussions. Additionally, there will be robust dialogue focused on workforce development in integrated care, financial and programmatic sustainability, payment considerations, research and evaluation, and the patient experience.
To achieve this goal, we will convene thought leaders across disciplines to discuss, debate, and take action on the most pressing issues necessary to bring about greater adoption and sustainability of integrated care. This will be an action-oriented conference, structured with multiple opportunities to make connections and participate in sessions designed to examine the current state of integration and look beyond, to create solutions for a connected, sustainable future.
More than 500 attendees are expected to attend the 2017 CFHA Conference in Houston, Texas.
