Participants must complete an attendance/evaluation form in order to receive a certificate of completion/attendance. Your chosen sessions must be attended in their entirety. Partial credit of individual sessions is not available.

How to Get Your Certificate for Continuing Education for Mental Health Professionals

1.       Go to the Amedco Attendance/Evaluation Link: http://cfha.cmecertificateonline.com/

2.       Click on the “18th Annual Conference” link

3.       Select all of the sessions you attended

4.       Evaluate the sessions and the conference

5.       A certificate will generate for you

 

The Amedco online form must be completed no later than November 15, 2016.

Research and Evaluation Training Track Certificate

Those who attend and sign in for two or more sessions in this track will receive a CFHA Research and Evaluation Training Certificate as recognition for their efforts to gain skills in research and evaluation methods. Get more info here.

This Live activity, Celebrating the Many Faces and Places of Integration, with a beginning date of 10/13/2016, has been reviewed and is acceptable for up to 19.25 Elective credit(s) by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
