2013 Conference Planning Committee

The committee is already at work to make the 2013 Conference in Denver a remarkable event.  There will be four distinct work teams to address key components of the Conference:

  • Health Policy Summit - led by Ben Miller and Cassidy Smith
  • Program - led by Laurie Ivey and Amy Davis
  • Social and Networking - led by Samantha Monson and Laura Sudano
  • Fundraising -  led by Polly Kurtz

The Planning Committee will meet as a whole three times prior to the Conference: November 14,  2012; January 23, 2013 and another time yet to be determined (possibly June 2013). Work teams will meet as needed throughout the year to accomplish their tasks.

Commercial Disclosure

The Collaborative Family Healthcare Association requires that all individuals who may affect the content of a continuing education activity must disclose any relevant financial relationships during the past 12 months with commercial interests. Relevant financial relationships also include relationships of a spouse or significant other. 

The disclosure form must be completed by:

  • CFHA Board of Directors 
  • Conference Planning Committee members
  • Conference proposal reviewers
  • CFHA staff and advisors
  • Conference presenters and co-presenters
