Survey re: OT as a member of the interprofessional primary care team

Occupational therapists (OTs) meet an array of needs for a variety of populations across multiple settings, one of which is providing services to patients in primary care offices.  We are investigating the perceptions of OTs and Primary Care Providers regarding the inclusion of OTs in this setting.
